Agreement

By using Medication List Template (the "site"), you agree to these terms. If you do not agree, please do not use the site.

What the site does

The site lets you create, print and download medication lists, logs, schedules, administration records and wallet cards, and read general information about medication abbreviations. Making, printing and downloading your list is free. Family Care is an optional paid subscription that sends medication reminders by email and alerts family members when a dose isn't marked as taken.

Not medical advice

The site is an organizing tool. It does not give medical advice, and it does not replace your doctor, pharmacist or prescription label. Please read our disclaimer.

Your responsibilities

• You are responsible for the accuracy of the information you enter and for checking printed lists against your prescription labels.
• Use the site only for lawful purposes and do not try to disrupt it, access it in unauthorized ways or misuse it.
• If you create an account, keep your sign-in details secure.

Your information

The free list maker stores your list in your own browser. Family Care stores the information you enter in your account. How we handle it is described in our privacy policy and consumer health data privacy policy.

Family Care subscription

• Price and billing. Family Care costs $4.99 a month or $35.88 a year, plus any tax shown at checkout. It renews automatically at the end of each period and is charged to your payment method through Stripe until you cancel.
• Cancel any time. Cancel under Settings → Billing. Reminders stop and you won't be charged again. We don't give partial refunds for the rest of a period unless the law requires it.
• Price changes. We will email you at least 30 days before a price change takes effect, and you can cancel before then.
• Who you add. Only add people you care for, and only email addresses whose owners expect to hear from us. Each recipient confirms before getting reminders or alerts and can stop them at any time.

Reminders are a helpful backup, not a safety system

Email can be delayed, filtered as spam or not delivered, and a dose can be taken without anyone tapping I took them. Family Care does not monitor anyone's health and is not an emergency service. Don't rely on it as the only way to make sure medicines are taken, and in an emergency call your local emergency number.

Our content

The site's design, text and templates belong to us. You may print and share lists and documents you make with the site for personal and caregiving use.

No warranty

The site is provided "as is" and "as available". We work to keep it accurate and online, but we do not guarantee that it is error-free or always available.

Limitation of liability

To the fullest extent allowed by law, we are not liable for any indirect or consequential loss arising from your use of the site, or for decisions made based on information you entered or printed.

Ending your account

You can delete your account at any time under Settings → Profile. That cancels any subscription and permanently deletes the information in it. We may suspend or close accounts that misuse the service, for example by sending emails to people who did not ask for them.

Changes

We may update these terms. The date at the top shows the latest version. Continuing to use the site after a change means you accept the updated terms.
