About Medication List Template
Who we are, why we built a free large-print medication list maker, and how we keep your information private.

What this site is
Medication List Template is a free tool for making a clear, large-print list of the medicines you take. You type each medicine once, then print it or download it as a PDF, Word or Excel file. There is nothing to install and no account to create.
We also offer printable versions for caregivers: a medication log, a medication administration record (MAR), a medication schedule, a wallet card, and a plain-English guide to prescription abbreviations.
Why we built it
Keeping an up-to-date list of your medicines is one of the simplest things you can do for your safety. Health agencies recommend bringing one to every appointment and keeping a copy where family can find it. Many people still write their list by hand, or keep it in their head.
We built this site for older adults and the people who care for them. That is why the text is large, the colors have strong contrast, the buttons are big, and you can make the text even larger with the A+ and A++ buttons at the top of every page.
Your privacy
The list you make stays in your own web browser. We do not see it, store it or sell it. You can read the details in our privacy policy.
Health information
Our guides explain things in plain words, but they are not medical advice. Always follow the directions from your doctor or pharmacist and on your prescription label. Read how we write and check our health content in our medical review policy, and see our disclaimer.
Family Care
Family Care is an optional paid plan that sends dose reminders by email and lets a family member know when a dose is missed. See pricing for details. Making and printing your list will always be free.
